Multiple
site refers to laboratory sites operated by the same corporation/entity
and meeting the following criteria, without exception:
- All sites must have the same Medical Director and Technical
Director.
- All physicians performing and/or interpreting any examinations
at any of the sites must be included in the application for
accreditation in the Organization section.
- All technologists performing any examinations at any of the
sites must be included in the application for accreditation.
- All physicians and technologists from all sites must participate
together in quality assurance and education programs, including
in-house conferences.
- Two additional cases per site must be submitted for
review following same case selection guidelines listed in the application.
ADDING A MULTIPLE OR ADDITIONAL SITE
Laboratories wanting to add a multiple site or an additional site to an application that has already been granted accreditation must submit the following:
- Complete the Multiple site/Mobile service supplement.
- The laboratory will need to complete the equipment section of the New/Additional Staff and Instrumentation Form. If any physicians and/or technologists have been hired since accreditation was granted at the main site, it will be necessary to complete and submit staff forms and supporting documentation for these staff members as well.
- The new site will need to complete the Add-on Site Audit materials (Attached to the end of the multiple site/mobile service supplement).
Upon receipt of all required materials, the accreditation decision will be made at the next monthly ICANL Board of Director's meeting. If the added multiple site or mobile service is accepted into the laboratory's accreditation, accreditation for the added multiple site service will expire when the laboratory's original accreditation expires. |

THINGS TO KNOW IF YOU ARE A MULTIPLE SITE LABORATORY
If eligible, the multiple sites may apply with a single
application and will be assessed an additional $750 per site (over one).
The
accreditation decision will be made for each site individually.
Multiple
site laboratories that are granted accreditation will receive
certificates for each testing area and each site.
All
ICANL correspondence will go through the laboratory listed
on the accreditation agreement. |
