So
you've come this far. You have filled out the entire application,
chosen the appropriate cases, and are now ready to send your
application to the ICANL. But perhaps you may still be a little
confused or have questions on how to actually submit your application.
The ICANL is here to help put your fears to rest, with the following
guidelines that should help you in submitting your application.
TWO COPIES OF THE APPLICATION
First,
you will need two (2) complete copies of your paper application.
This includes all of the application paperwork plus the attachments
and case studies (with reports, hard copy images in black-and-white
and color, worksheets, ECG tracings and stress details, as applicable
to the Application for Nuclear Cardiology, General Nuclear Medicine,
or PET). Place the attachments behind the page on which the
question is asked. Submit only those attachments that are requested
on the Attachment List found on the last page of each section.
Please separate Set 1 of the application from Set 2. To separate
the application, use large envelopes, rubber bands or binder
clips. Do not place the paperwork inside 3-ring binders or
use page protectors, as the application will be separated
once it is received by the ICANL.
Make
sure you keep a complete copy of the application, including
attachments and case studies, for yourself.
SUBMIT DIGITAL DATA
Only
one (1) copy of the digital data is required for submission.
All digital data should be submitted on CDs, floppy disks, optical
disks, tape cartridges, or by internet via FTP. The digital
(raw, unprocessed) data is for the cardiac cases only. We are
currently unable to load General Nuclear Medicine and/or PET
data on the Core Lab site. If your system is capable of creating
a CD with JPG or TIF images, or an AVI file, the data
from General Nuclear Medicine and/or PET cases may be submitted
in duplicate. If you have any questions regarding the submission
of the cardiac digital data, please call the Yale Core Laboratory
at (203) 785-5005.
ACCREDITATION AGREEMENT
It
is also necessary to include an original signed Accreditation
Agreement. The current Agreement can be downloaded from www.icanl.org.
Check the revision date to ensure that your laboratory is working
with the most recent version of the ICANL Agreement that is
dated 3/04. If submitted on an earlier version, the Agreement
will be returned and could delay your final decision notification.

MAILING
It
is strongly recommended that your laboratory choose a trackable
delivery method (i.e., UPS, FedEx, or any delivery company
that assigns a tracking number to your package) to avoid having
the application get lost in the mail, or worse -- disappear
without a trace.
Remember,
you've made it through the tough part, and your many hours of
hard work will pay off. By following these few simple steps,
you are now ready to close up the box and submit your application
with a feeling of accomplishment and the confidence of knowing
your facility is one step closer to becoming an accredited laboratory.
