As
reported in the May issue of the ICANL Newsletter, all applicant
laboratories, by way of a random selection process, are either
selected to receive a site visit or required to submit specified
policies and supporting documentation for an audit.
Shortly
after each submission deadline, the ICANL notifies the applicant
laboratories of which process they will undergo: the site
visit or the audit of policies and documentation.
What happens if my lab is selected for an audit?
Those
laboratories selected for the audit are provided with an audit
form to be completed, signed and returned to the ICANL office,
inclusive of specified attachments, within 45 days. The required
questions and attachments are repetitive of the details reviewed
during the site visit and include aspects that are not part
of the application.
Laboratories
selected for the audit process are required to provide data
and submit attachments related to the following areas:
- STRUCTURE
AND ORGANIZATION
- Ancillary
Personnel
- Physical
Facilities
- Stress
Procedures
- Equipment
and Instrumentation
- PROCEDURES
AND PROTOCOLS
- Imaging
Equipment
- Non-Imaging
Equipment
- Radiation
Safety and Radioactive Materials Handling Protocols
- Administrative
and Other Protocols
- QUALITY

The
ICANL's Random Audit Review Committee is comprised of peer reviewers
from accredited laboratories. Their evaluations of the audit
materials, along with those of the two independent application
reviewers, are reviewed by the Board of Directors prior to the
rendering of each laboratory's accreditation decision.
