FEE
FOR ACCREDITATION MATERIALS
The
one-time fee to purchase the printed version of The Standards
and the Electronic Application, is $200. Notification of
periodic updates to The Standards are automatically sent,
free of charge, to every laboratory that places an order. Ordering
the ICAVL Accreditation Materials does not obligate the laboratory
to apply for accreditation within any given period of time.
APPLICATION
FEE
Application
fees include a base fee of $1000 and an additional $300
for each area of accreditation being sought. Based on the testing
procedures offered within their facility, laboratories may apply
in any combination of the following areas of accreditation:
- Intracranial
Cerebrovascular
- Extracranial
Cerebrovascular
- Peripheral
Arterial
- Peripheral
Venous
- Visceral
Vascular
Any
laboratory with more than one site may be eligible to apply
for multiple site accreditation, and will be charged
$750 for each site. (The base site is excluded from this
fee.) Please review the Multiple
Site Policy and contact our office with any questions.
Note:
Application fees should not be paid until the laboratory applies
for accreditation.
