A
mobile service is comprised of one or more units (technologist
and equipment) that provide vascular testing services at one
or more locations and meet the following criteria, without exception:
1. The entire mobile service must share the same Medical Director.
2. The entire mobile service must be supervised by the same Technical Director, who is responsible for ongoing evaluation of the technical component of the testing performed by the mobile service.
3.
All medical staff interpreting mobile vascular examinations
must be included in the application for accreditation.
4.
All technical staff performing mobile vascular examinations
must be included in the application for accreditation.
5.
All physicians and technologists must participate together
in quality assurance and education programs, including in-house
conferences.
6.
The entire mobile service must utilize identical protocols.
7.
The entire mobile service must utilize the same diagnostic criteria.
8.
The entire mobile service must utilize the same level of equipment for all testing.

THINGS TO KNOW IF YOU ARE A MOBILE SERVICE LABORATORY
If eligible, applicants with a mobile service may apply
with a single application and will be assessed a $750
mobile application fee.
If
accreditation is granted, it will be granted to the mobile
service collectively, and not to each location individually
(although the name and address of each location will be
entered into the ICAVL database).
Applicants
with mobile services that are granted accreditation will
receive one certificate, listing the institution name as
listed in the accreditation agreement, for each testing
area.
All
ICAVL correspondence will go through the laboratory listed
on the accreditation agreement. |

ADDING
A MULTIPLE SITE OR A MOBILE SERVICE
Laboratories wanting to add a multiple site and/or mobile service to an application that has already been granted accreditation must submit the completed multiple site / mobile service supplement and an updated equipment table (if applicable). If any physicians and/or technologists have been hired since accreditation was granted, it will be necessary to contact the ICAVL office regarding the submission of staff forms and representative cases for these staff members. In addition to the multiple site / mobile service fee, there may be an added fee to review the staff forms and representative cases. If the added multiple site or mobile service is accepted into the laboratory’s accreditation, accreditation for the added multiple site or mobile service will expire when the laboratory's original accreditation expires. |
